The budgeter is mainly oriented to OpenEMR – OpenEMR is an open source, free software for medical records. It fulfills all the security and quality requirements for HIPAA and other public agencies in the US. It supports the HL7 data exchange standard, exporting “Continuity of Care” documents with all the patient data, in a standard, compatible format.

The page: Download OpenEMR contains the main links. Our clients receive the updated terminology table, in international Spanish, from which we have refined almost 20 unnecessary languages. We also have teaching materials in .pdf and PowerPoint formats, with the main tasks performed by users (doctors, nurses or administrative). The Windows version is 43.8 mb. It requires to run an Apache server emulation, with PHP and MySQL. The complete version, with the XAMPP system (Apache emulator, PHP, MySQL and mail), has 227 mb and can be downloaded from the aforementioned page. If you have a Linux system, for example Debian or Ubuntu, download your version (of 33 mb) on the Oem.org project site.

After installing the software on a local area server, which can be any Windows machine, perform the tasks of configuration and adaptation to local needs. These tasks include definition of care centers, professionals in charge of medical services, opening hours, health insurance available, etc. Regarding the system itself, there are many modules that are enabled or disabled according to the needs of users. If you are not an expert in this type of computer-medical-administrative task, ask about our initial configuration service, training and support.

Medical appointments system with online appointment calendar

The free open source system OpenEMR has a medical appointment system with calendar, as an integral part of the software, which can also be used separately from the Digital Clinical Records function. You can simply ignore everything that is not needed, and limit yourself to the functions of administering users of the system (user and password), awarding care appointments to professionals providing services, and arranging appointments with patients .

To register a Patient in the system, you need the date of birth, sex and a first and last name. You can add, if desired, a huge variety of demographic data, health insurance, clinical, contact, etc.

To register a Medical Center in the system you only need a name, and a series of location, contact data, if it is a billing center, and even identification color on the screen are optional.

To register a Provider (in general, doctor), user name, first and last name, password, Center by default where it attends and level of access are required, this being maximum (Administrator) or minimum (Clinical). In the same User registration screen you must specify if the same is a health service provider, if it has a presence in the Calendar and if it is active. Access levels that are not usually health service providers or have access to the calendar are the Accountant and the receptionist (deskfront attendant).

To record an Appointment between (Patient and Provider), click on the desired time, within the column of the desired provider, and select the patient. Then the appointment is confirmed.

After registered, the Appointment can be edited to change some data, or it can change of State, becoming: Canceled, Absent, Arrived, Arrived late, Retired without consultation, etc.

Both the programming of hours of presence in the Center for providers, as the Appointments for patients, can be repeated in days, weeks or consecutive or alternate months with a couple of clicks.

The Administrator / Reports function can issue reports of Appointments, Meetings, Appointments without Meeting, etc., for one or several lenders and / or Centers.

If the Patient Portal is activated, it can see on the online site its future appointments with professionals, among other data.

In summary, OpenEMR’s appointment scheduling and meeting schedule function is extremely complete, and covers the needs of most clinics and medical centers.

If the user wishes, the system functions that will not be used can be disabled from the menus, in order to further simplify the task of the system operators.

Free software installation for electronic medical records: OpenEMR

The software for clinical history can be proprietary or public, depending on the type of license adopted by the author. In addition, they may have cost or be free. OpenEMR is a clinical history computerization solution that offers the greatest advantages in terms of cost, ease of use, support and security, and has the advantages of being public and free.

OPENEMR is an international project, with more than 45,000 installations in all the world, and hundreds of active developers. Because of its size, it has more than 1000 features,  Openemr has many features, well above most commercial systems. The OpenEMR project is managed by Sourceforge.net, the platform more widespread for open source and free projects.

Ejemplo de facturación y contabilidad

Veamos un ejemplo de práctica médica facturada.

Lo primero es seleccionar al paciente. Al hacerlo, se habilitará la opción de “Crear una visita”. En  la visita se debe especificar si es Paciente Nuevo, Paciente Establecido u otras opciones. También se podrán agregar los Incidentes que correspondan: Problema – Alergia – Medicación – Cirugía – Dental. 

Al abrirse la Visita se habilitará la opción Hoja de tarifas en la opción Tarifas del menú de navegación. Se utiliza mucho el sistema de códigos ICD9 o ICD10, los cuales están en inglés. Es conveniente utilizarlos, ya que muchas aseguradoras de salud exigen su uso para validar los pagos. En la versión 4.3 OpenEMR anuncia la incorporación del SNOMED CT, base de datos de términos clínicos perteneciente a una entidad supranacional sin fines de lucro, la International Health Terminology Standard Development Organisation (IHTSDO).

SNOMED tiene una versión en español. Cada usuario debe solicitarla en el sitio de la National Medicine Library del National Institutes of Health (Washington). Pesa 245 mb.

En OpenEMR 4.2 se presentan por defecto 3 opciones para Códigos de Pago:

  • Diagnóstico ICD9 (International Classification of Diseases)
  • Procedimiento/Servicio CPT4 (Current Procedural Terminology)
  • Procedimiento/Servicio HCPCS (Healthcare Common Procedure Coding System)

Los procedimientos (no los diagnósticos) a menudo vienen con un precio o tarifa sugerido.

Guía del Usuario OpenEMR 4.1- Manual práctico

Hemos publicado la versión libre y gratuita de la Guía del Usuario para OpenEMR. Corresponde a una traducción de la versión 4.1, que no difiere mucho de la actual. Está anunciada en el Wiki de OpenEMR, http://www.open-emr.org/wiki/index.php/Spanish_Guide , para que todos los usuarios hispanos puedan aprovecharla y eventualmente ayudar a mejorarla.

Además, disponemos de una versión ampliada, con muchos temas que no están en la fuente inglesa mencionada, pero que son necesarios para el buen aprovechamiento del software. Esta versión es ofrecida a nuestros clientes, en particular al Instituto Inec en Colombia, que dicta cursos al respecto. Solicite esta versión de nuestro Manual de OpenEMR en español si Ud. tiene intención de instalar OpenEMR en su institución.

Como parte del paquete de ayuda para los alumnos, hemos puesto en marcha un sistema de tickets, en este sitio. El soporte por tickets es mucho más ordenado que los emails sueltos. En primer lugar, el usuario puede llenar casillas en lugar de escribir creativamente. Y eso representa ahorro de tiempo para el que escribe y también para el que lee. En segundo lugar, los distintos temas pueden rutearse automáticamente a distintos técnicos. En tercer lugar, queda un registro de cuando y cómo fueron respondidos los mensajes.
